Normal resident duty hour/backup schedule

The department of plastic surgery call schedule meets all California legislative mandates and RRC requirements regarding resident call. There is no in-house call. Conceived by the resident staff and agreed to by the attending staff, a program-wide call schedule was adopted July 1, 1994. This call schedule provides not only clinical coverage but also allows less weekend call than prior schedules and provides greater opportunities for rest, relaxation, and study (see below). In addition, resident backup coverage was added.

The Kaiser Permanente Hospital/Fontana (KPH) junior plastic surgery resident will share call with one or two general surgery residents so that night time call responsibilities will be every third night. Call may be taken from home. There is no inter-hospital cross coverage during this rotation.

Vacation/meeting schedule

The division of plastic and reconstructive surgery will continue to follow the various recommendations of the graduate medical education LLUMC House Staff Manual regarding resident benefits, vacation, meeting time, etc. Currently, residents will have a minimum of four weeks (20 working days) vacation each year. Time will be separate from that taken to attend academic meetings, subject to approval by the rotation director and the residency program director. Provision will be made for cross coverage when residents elect to be away on vacation or at meetings.

Research

Residents in plastic surgery will be encouraged to become involved in a basic science or clinical research project early in their residency so that ample time will be available to complete a project. Specific time devoted strictly to the research project will be assigned as an integral part of the rotation at the Jerry L. Pettis Memorial Veterans Medical Center. Nearly 30 percent of the time spent on this rotation will be assigned to non-clinical endeavors.

The research will be conducted under the supervision of one of the faculty members within the division of plastic surgery. Exceptions will be made if a collaborate effort with an investigator outside of plastic surgery is arranged. The research topic will be of the resident's own choosing and the project will be overseen by the designated faculty advisor. Basic research; as well as chart reviews and/or unique case reports, are all acceptable for research. Funding for projects will be provided through division funds or grant monies.

There is a dedicated plastic surgery lab with 594 square feet. This lab has recently been reorganized to adapt to changing technology. We have recently recruited Dave Warner to coordinate these activities. Each resident is given ample time (Rotation #3 - VA/research) and is expected to complete either a basic science (preferable) and/or clinical research project during his/her residency. It is expected that he/she will submit and present his/her completed project at the annual senior residents conference. The major emphasis of this lab will be on fetal wound healing, intrauterine surgery, and microvascular surgery. We have planned for two full-time lab technicians.

The dedicated microvascular teaching laboratory also lend itself to ongoing research studies; e.g., blood flow monitoring, secondary ischemia time and its relations to flap survival, and the effects of long-term smoking.
