MITHS
Before starting, be sure to have all of the following items:

- Student contact information
- Proof of citizenship (copy of birth certificate, passport, or resident card)
- Guardian/parent - Emergency contact information
- Health insurance information (Company, policy holder, and policy number)
- Professional photo to upload
Academic Information
- School contact information
- High school transcripts 9th, 10th, 11th (first semester transcripts) (cumulative 3.5+ GPA)
- PSAT, SAT, or ACT score results Awards & Achievements
- List of honors, awards, and election leadership positions
- List of extracurricular activities
- List of work or volunteer experiences
- List of relevant course not on transcripts
Essays (each 300 words or less)
- Essay 1 - Write a brief autobiography essay. Include a description of why you want to be a part of the MITHS program, what sparked your interest in the health sciences, your chosen extracurricular activities and your choice of professional endeavors.
- Essay 2 - Describe a mentor (non-parent/relative). How has this person’s career contribution inspired or affected you?
Three recommendations
- One Academic Reference (science or math teacher)
- One Guidance Counselor Reference (guidance counselor or principal)
- One Pastoral Reference (SDA church pastor or chaplain)
MITHS participants will be selected from a pool of Black applicants who meet the following criteria:
- Junior in high school
- Overall GPA of 3.5+
- High academic performance in science and math courses
- Seventh-day Adventist Church membership or affiliation
- Citizenship record in good and regular standing
- Participation in leadership, school activities, and community service
- Strong interest in a career in the health sciences
